The class III histone deacetylase sirtuin 6 (SIRT6) modulates numerous functions in the cell by deacetylating histone lysine residues. Interestingly, SIRT6's efficiency in in vitro experiments is far greater against substrates carrying long-chain fatty acyl modifications such as myristoylated lysine compared with acetylated counterparts, but the deacetylase activity can be stimulated by fatty acids and small-molecule allosteric modulators. A new study helps to explain this puzzling activation using a novel activator, thorough kinetic investigation, and mutagenesis studies. These data help elucidate the molecular requirements for activation of SIRT6 and provide a foundation for development of activators for therapeutic purposes. © 2020 Nielsen and Olsen.BACKGROUND Previous studies have indicated that higher subjective well-being would works as a protective factor for health. Some studies have already shown Mindfulness-Based Intervention's effects on improving subjective well-being. However, they targeted specific populations rather than the general public. Furthermore, they assessed either the life evaluation or affect aspect of subjective well-being, rather than the wholistic concept including eudemonic aspect of well-being. Selleck Raptinal OBJECTIVE The objective of this study is to investigate the effectiveness and cost effectiveness of Mindfulness Based Cognitive Therapy (MBCT) for improving the wholistic aspects of subjective well-being on healthy individuals. METHODS This study is eight-week randomised, parallel-groups superiority trial with two-month follow-up. Healthy individuals aged 20-65 years with scores lower than 25 on the Satisfaction With Life Scale will be eligible participants and randomly allocated to MBCT, or wait-list control. The intervention program is the modified version of MBCT developed by arranging a MBCT program to improve the well-being of a non-clinical population. The primary outcome is the difference in mean change scores from the baseline on the SWLS between the two groups. The secondary outcomes include Flourishing Scale, Scale of Positive and Negative Experience, Incremental Cost Effectiveness Ratio etc. RESULTS This study began recruiting participants in July 2018. The study is still ongoing and data collection will be completed by December 2019. CONCLUSIONS This study is unique in that the it investigates MBCT's effects on the three different aspects of subjective well-being. Registered 27 March 2018.INTRODUCTION Soft tissue reconstruction around the knee is challenging from a functional and aesthetic perspective. While locoregional options remain limited, free flaps produce additional scarring and longer hospitalization. We describe the most distal "D-perforator" of ALT-axis and present our 4-year experience with a distal perforator-only propeller anterolateral thigh (D-POP ALT) flap for reconstruction around the knee. METHODS Seventeen patients (7 males, 10 females, mean age 57 years), had distal perforator-only propeller (D-POP) ALT flap reconstruction of defects following the wide local excision of melanoma around the knee joint between May 2014 and December 2018. The most distal perforator in the line between spina illiaca anterior superior (SIAS) and the upper lateral border of patella was identified and marked with audible-Doppler and perforator-only propeller ALT (POP-ALT) flap, which was designed around it. Perforators were dissected intramuscularly or intraseptally to allow adequate flap mobilization, but no division of main pedicle was ever performed to ensure anterograde blood supply.
